Please read this in full. It is a legal
agreement, and by signing it you give up certain rights.
1. Assumption of risk
I understand that WingTsun Kung-Fu and self-defence training are physical
activities involving contact with other people. Training includes striking and
blocking drills, partner work, close-range contact, falls, and use of training equipment.
I understand that these activities carry an inherent risk of injury — including but not
limited to bruises, sprains, strains, fractures, dental or eye injury, concussion, heat
exhaustion, cardiac events, permanent disability, and in rare circumstances death — and
that such risks cannot be eliminated regardless of the care taken by instructors.
I am voluntarily choosing to participate with full knowledge of these risks, and I
expressly assume all such risks, both known and unknown.
2. Release and waiver of liability
In consideration of being permitted to participate, I hereby release, waive and
discharge New Path WingTsun Kung-Fu Sciences, Shifu Ean Willis, its instructors,
assistants, students, officers, agents and volunteers (the "Released Parties") from any and
all claims, demands, actions or causes of action for loss, damage, personal injury or death
arising out of or connected with my participation, including claims arising from the
ordinary negligence of the Released Parties.
This release does not apply to conduct that is wilful, wanton, reckless
or intentional, and does not waive any right that cannot be waived under Illinois law.
3. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability,
damage or cost, including reasonable legal fees, arising from my participation, or from any
injury or damage I cause to another person or to property.
4. Physical condition
I confirm that I am physically able to participate, that I have disclosed any medical
condition, injury, allergy or medication relevant to safe training on this form, and that I
will inform an instructor promptly if that changes. I understand that I am advised to
consult a physician before beginning any new physical activity, and that the school is not
qualified to give medical advice.
5. Emergency medical treatment
In the event of injury or medical emergency, and if I am unable to be reached or unable
to consent, I authorise the Released Parties to secure emergency medical treatment,
including transport to a medical facility. I accept financial responsibility for the cost
of any such treatment.
6. Rules and conduct
I agree to follow all school rules and instructor directions, to train with control and
respect for my partners, and to refrain from training while under the influence of alcohol
or drugs. I understand that the techniques taught are for lawful self-defence
only, that misuse may carry criminal and civil consequences, and that the school
may suspend or remove any student for unsafe or inappropriate conduct.
7. Minors
If the student is under 18, I confirm I am the parent or legal guardian with authority to
sign on their behalf, and I agree to each provision above on my own behalf and on behalf of
the student, to the fullest extent permitted by Illinois law.
8. Governing law and severability
This agreement is governed by the laws of the State of Illinois. If any provision is held
unenforceable, the remaining provisions remain in full force and effect. I confirm that I
have read this agreement in full, that I understand it, and that I sign it freely.
